Frequently asked questions:
How early do I need to be at my appointment?
- Please be at our office 10-15 minutes ahead of your appointment time with your paperwork filled out. Make sure both sides of the forms are filled out and that every page is signed.
Do I need to pay at every visit?
- Yes. You may pay ahead for a couple of visits, but we do require payment upon service.
What is the HIPPA form?
- This form is simply to let you know that we keep your information private. The only people who can receive your information are those you list on the front of the form. For more information go to the "Links" section of our website and click on the HIPPA link.
What is your "No Show" policy, and do you enforce it?
- If you do not call ahead to cancel your appointment, you will be billed the full charge for that time. Our schedule is full, so please respect the time of our therapists. We know things can come up, so please call and let us know if you will not be able to make it to your appointment.
What type of clothes should I wear?
- This depends on what you are coming in for. If you are coming in for knee therapy, wear shorts or bring a pair with you to change into. If you are coming in to have a shoulder treated, wear or bring a tank top. If you have a concern, don't hesitate to ask if the thereapist would like you to wear something specific for therapy.
What should I bring with me to my visit?
- On your first visit, please bring an up-to-date insurance card, your prescription from the referring physician (if you have it), your filled-out paperwork (if you received it in the mail) and your payment for therapy. Your therapist will let you know if there is anything you need to bring with you to future appointments.
